Patients of Record: you may fill out an Appointment Request form below, but please be aware that you must call the office for immediate assistance. If your emergency is after business hours, there will be a recording on the office line with directions for how to proceed.
New Patients: our recommendation is to call the office for immediate assistance. If the office is not opened at the time of your emergency, we will be in touch with you on the next business day to help and set up a chart for you and rectify your dental emergency.
You may change an existing appointment 48 hours or more in advance by calling the office or filling out an Appointment Request form below.
Fill out the form below and someone from the office will contact you as soon as possible to answer any inquiries you may have and/or to provide you with your appointment specifics. We need two days notice for appointment cancellations submitted via our online portal. Please note, the dates and times you requested may not be available and we will always contact you to confirm your actual appointment details.
